Pages are rendered the way a PDF viewer shows them, on a white page background, so the output is a fully opaque image. PDF pages themselves don't have transparent backgrounds โ the white "paper" is part of the page.
High (216 DPI) suits most on-screen uses, including presentations viewed on large displays. Choose Maximum (288 DPI) for printing or when readers will zoom deep into small text; choose Standard (144 DPI) only when file size is tight.
It depends on the scan. Crisp black-and-white text scans keep their sharpness better as PNG. Scans that are essentially photographs โ color brochures, photo albums โ gain little from lossless encoding and end up far larger, so JPG usually serves those better.
